Benefit enhancements for Arizona dual special needs plan (DSNP)

The UnitedHealthcare Dual Complete^® plan is available for individuals who qualify for both Medicare and Medicaid. The plan has enhanced several benefits for qualified plan participants in 2021.

New or enhanced benefits for 2021:

• Food Allowance: $25-$50 per month on a debit card to purchase fruits, vegetables, dairy products, meats, breads and more at many retailers. New benefit.
• Prescriptions: $0 drug copays on all tiers of covered generic and name-brand prescription medications with option for home delivery. Removal of copays.
• Virtual Mental Health Visits: $0 copay virtual mental health visits to get a private evaluation and treatment for general mental health conditions. New benefit.
• Routine Chiropractic Care: Members can receive a up to 12 routine chiropractic visits per year for relief of nausea or nerve, muscle or joint pain. Increase of two visits annually, no longer shared benefit with acupuncture.
• Routine Acupuncture Care: Members can receive a up to 12 routine acupuncture visits per year for relief of nausea or nerve, muscle or joint pain. Increase of two visits annually, no longer shared benefit with chiropractic.
• Transportation: 36 one-way trips annually to plan approved locations. Increase of 12 one-way trips annually.
• Post-Discharge Meal Program Benefits: Members can receive up to 42 meals for 21 days, one time per year, after a hospital stay to maintain nutrition. Increase of 28 meals annually.
• Over-the-Counter (OTC) Debit Card: UnitedHealthcare will provide $290 quarterly/$1,160 annually on a debit card toward the purchase of over-the-counter products at many retailers or for home delivery. Increase of $60 in credits per year.   
• Dental: Up to $4,000 for covered dental services such as certain cleanings, fillings, crowns, root canals, extractions and dentures. Increase of $500 annually.
